Care Home Capacity Tracker
The Care Home Capacity Tracker is a digital portal designed by the North East Commissioning Support Unit in partnership with NHS England with the aim of making the discharge process into care homes more efficient and improving a patient’s transfer of care.
Please note that during the COVID-19 outbreak, the CCG and partner organisations are requesting that providers update the tracker each day (by 9:00 am if possible), this is to ensure the information we have is as up to date as possible to help with expedited discharges. A section to monitor the effect of the outbreak on care providers has been added to the tracker, please update the "Business Continuity" section daily (CQC are asking all providers to report in through this mechanism, with the data going into national reporting as well as being accessed by the CCG and local authorities to help provide support to providers) The scope of the tracker has also been widened during this period to include learning disability homes, community units, community rehab units and hospices. The tracker allows care homes to upload information about:
- The number of beds they have available to admit to;
- The type of beds available
- The cost of the beds
- Any useful information about the services that are offered at the home.
This allows our care partners to locate beds more efficiently and find care that is the most appropriate to the individual’s
The system runs efficiently thanks to our care home partners regularly updating their bed figures and availability. The tracker is very quick and easy to use and it takes just 30 seconds to update. One of the greatest benefits is that our healthcare provider partners save an enormous amount of time in trying to find suitable care, as well as saving avoidable calls to care providers. The CCG, in collaboration with partners at North Yorkshire County Council, East Riding of Yorkshire Council, City of York Council and York Teaching Hospital NHS Foundation Trust, is aiming to implement the tracker in all in scope care homes in the Vale of York.
