The Vale of York Proactive Health Coaching initiative has proudly been chosen as a finalist for acute sector innovation in the prestigious HSJ Value in Healthcare Awards 2017.
Proactive Health Coaching is a telephone-based health management service that improves patient health and quality of life, while helping to ensure healthcare resources are spent as efficiently as possible. In partnership with Health Navigator and York Teaching Hospital NHS Foundation Trust, we have delivered an effective preventative strategy that provides better care for patients and reduces stress on A&E departments.
Fiona Bell, the CCG’s Innovation Lead said: “We are thrilled that our Proactive Health Coaching project has been shortlisted, especially among such stiff competition.
“The project; a telephone based health management service, is a fantastic example of commissioners, providers and private sector partners working collaboratively across what can be complex organisational boundaries and really making a difference to our patients and communities.
“There’s some brilliant work going on in the system and I’m delighted that work to build confidence with patients to manage complex conditions and improve their quality of life is being recognised.”
Proactive Health Coaching facilitates:
- commissioners, acute providers and primary care being able to work together;
- hence a better quality of life for patients;
- a reduction of the burden on A&E and inpatient services;
- a more efficient use of health care resources.
